Old Moon Guild's Manager Jetina NPC Knowledge says you will need the Oquilla Sky Crystal to craft the Resplendent Weapon Reform Stone V to reform a Resplendent weapon to PEN (V). Bring her Oquilla Piece of the Old Moon x70 to obtain one.

※ You cannot proceed with this quest while having accepted "Resplendent Armor Reform Stone."
※ You can purchase Oquilla Piece of the Old Moon from the Old Moon Shop of <Old Moon Manager> NPCs located in major cities.
※ The Oquilla Sky Crystal awarded by this quest is used to craft the Reform Stone V to upgrade a Resplendent main/sub-/awakening weapon to reform level V.
